Paola Forman passed away Nov. 21, 2020.

She was born on March 6, 1990, to William and Monica Chunga and attended Putnam City North High School.

She married Eric Forman on Aug. 6, 2016, and they had three beautiful children.

Arrangements were under the direction of Sanders Funeral Service, Kingfisher.
